I think you missed your chance with 47. Rh5 chasing the Black a-pawn the wrong way. You should have instead attacked the pawn more directly with 47. Ra6! bringing the Rook behind the pawn so that it cannot escape. And if Black takes your h3 pawn with his Rook you take the a5 pawn with your Rook. And then if Black takes your g4 pawn with the King then you play your Rook to d5 winning Black's last pawn for a draw.
In Rook and pawn endgames it is all about the activity of the respective Rooks and your Rook would be more threatening and active on the a6 square than the h5 square where you played it to giving Black time to escape with the pawn.

I actually took 20 minutes to make move 47...and I miscalculated Ra6 but DID look at it. Thank you Matt!
